About the Award

The Jon Hilton Award for Excellence in Primary Care Informatics is awarded to a person or organisation who best personifies the priorities for which Jon was such an advocate. These include innovation in General Practice, Care Co-ordination, Electronic Referral and Chronic Disease Management.

Jon Hilton was a major contributor to HISA for more than 20 years commencing in the early days in the HISA Office with Joan Edgecumbe.

Background

Jon Hilton was a major contributor to HISA for more than 20 years commencing in the early days in the HISA Office with Joan Edgecumbe. Jon was a passionate Health Informatician who was clear in his priorities for improvements in the health system. Jon’s commitment was enhanced by his early experience in working in a General Practice clinic and in creating innovative Primary Care systems.

Jon has also been a passionate supporter of HISA and was a Member of the HISA Board, the Victorian State Branch Committee and represented HISA on a number of Standards bodies.

Jon was a strong advocate for better communications between GPs, Allied Health practitioners and acute hospitals. He was a major player in the introduction of Care Co-ordination systems into Victoria which led to him creating an Australian Standard for eReferral in AS4700.6 – Referral, Discharge and Health Record Messaging. He spent many hours promoting this standard at an international level.

Jon’s passion for better communications with primary care providers ultimately led him to join with Precedence Healthcare to expand, support and promote the use of the cdmNet System for Chronic Disease Management. Having a chronic disease himself, Jon fully appreciated the degree of fragmentation of vital health information and he worked hard to improve this complex environment.

Jon always fought for empowerment of patients through better access to their health information and for a better understanding by health practitioners that fully-informed patient consent was paramount. This campaign was exemplified by Jon’s documentation of the disease that ultimately ended his life.

Purpose of this award

The Jon Hilton Award for Excellence in Primary Care Informatics is awarded to a person or organisation who best personifies the priorities for which Jon was such an advocate. These include innovation in General Practice, Care Co-ordination, Electronic Referral and Chronic Disease Management.

What does the Scholarship provide?

The Jon Hilton Award will be granted to the successful applicant comprising of a cash prize plus complementary registration at the HIC conference, including admittance to associated events, the networking reception and the HIC Gala Awards Dinner.

Any other associated costs (e.t. travel and accommodation) are not included.

Eligibility

The Jon Hilton awardees will be selected from nominated individuals or organisation. The Jon Hilton awardees will be selected from nominated individual or organisation. Nominations must be submitted for practitioners who have created an innovative development that will improve co-ordination of health information and assist in clinical decision-making in primary care. The innovation or development should support delivery of a sustainable outcome that has applicability across the primary care sector.

Selection and Review Process

Nominations will be co-ordinated by the HISA National Office with a review and recommendation process by a specialist group of health informaticians with expertise and interests in Primary Care.

The Award is made available at the discretion of the HISA and the Reviewers.
